NO  NEEDLE,  NO  SCALPEL TECHNIQUE

Pictureclick to enlarge
No needle:  The no-needle technique has made vasectomy nearly painless.   Instead of using a needle and syringe to numb or anesthetize the scrotum, we use a spray applicator called a MadaJet to apply the numbing medication.  Although not completely sensation-free, many men compare the feeling of the spray applicator to the that of a rubber band snap.

No scalpel:  Instead of making two incisions in the scrotum as is done in the traditional vasectomy technique, we only make one small hole in the scrotum for the entire procedure.  The area of skin is completely anesthetized by the Madajet.  The hole is so small that stitches are not needed afterwards.

What to do:
  • Understand the alternatives to vasectomy so that you are confident in your choice.
  • Understand the "Post-Vasectomy Instructions" so that you know what to expect.
  • Please shave the front wall of the scrotum and shower the hair away before coming in.  (See example pic)  The doctor can do this for you on the day of the procedure if you feel uncomfortable doing this.
  • Eat a normal breakfast or lunch before your procedure.  You do not need to fast.  Nervous men who do not eat beforehand are more likely to become lightheaded during or after their vasectomies.
  • We strongly recommend that you arrange to have someone drive you home.  Some men may experience delayed light-headedness following a procedure.
  • Plan to do nothing but recline at home on the evening of the vasectomy.
What NOT to do:
  • Do not use powder or deodorant in the genital area on the day of your procedure.
  • Do NOT take any aspirin or aspirin-containing products for 7 days prior to your procedure. Tylenol (aceteminophen) and Advil (ibuprofen) are OK to take.
What to bring on the day of your vasectomy:
  • Your insurance card and photo ID.
  • Payment for the procedure.
  • Wear underwear that is snug, but not uncomfortable.  Briefs, boxer briefs, and athletic supporters are all fine.  Do not wear boxers.  You should wear this into the clinic on procedure day.

  • On the day of your vasectomy, spend a quiet evening at home, reclining in bed or on the sofa. Minimize activity.
  • Avoid aspirin for 2 days after the vasectomy. You may take acetaminophen (Tylenol) or Ibuprofen (Advil) if you have any discomfort.  Ibuprofen is generally preferred if this is something you can take.  We recommend taking 600mg every 6-8 hours.
  • Ice packs may be helpful.  Though if you don't find them helpful, don't use them.  It does help improve the sympathy factor, which may help you maximize the benefits of your vasectomy weekend.
  • You may remove your supportive underwear and take a shower starting the morning after the procedure.  You should otherwise wear the supportive underwear all of the time for the next 7 days, even while sleeping.  This will help decrease swelling and achiness.
  • On the day after the procedure, you may walk and drive as much as you like, but no sports, yard work, swimming, or heavy lifting for 7 days.  After 7 days, there are no restrictions but it is recommended to slowly ease back into your regular activities. If your job is sedentary (office work or supervisor), you may return to work immediately.  If your job involves physically strenuous activity, you may want to consider taking a couple of days off of work.  We can provide you with a ‘doctor’s note’ if needed.
  • The American Urological Association Vasectomy Guidelines recommend that men refrain from ejaculating for one week post-vasectomy; however, a couple of days is probably adequate if you have no pain.  Blood in the semen within the first month or two after the vasectomy occurs in some men, but is not reason for concern.
  • No follow-up visit is required, other than the post vasectomy semen analysis, and this is something you will mail back. However, you will be given Dr. Zola’s cell phone number and if you have undue discomfort, or any concerns, you should call and speak with him and arrange follow-up if necessary.
  • It is normal to have some discoloration of the skin (black and blue) around the puncture site a day or two after the vasectomy. Some men will develop considerable discoloration of the scrotum about 4 days after the vasectomy. Blood from the deep vasectomy site comes to the surface as a purplish-blue mark, gets darker, and spreads out like an oil slick, then gradually dissipates.
  • Some men (about one in 20) will develop swelling and discomfort on one side, sometimes on both sides, starting anytime from 3 days to 3 months following vasectomy. This usually represents an exaggerated form of the normal inflammatory response necessary for sperm resorption and recycling. It is effectively managed with a 5-7 day course of ibuprofen 600 mg 3 times per day.
  • Twelve weeks and thirty ejaculations after your vasectomy, have your semen tested to be sure that it no longer contains sperm and that it is thereby safe to stop other forms of contraception.
